Our Qualified Mental Health Professionals provide goal-directed training to enable individuals to achieve and maintain community stability and independence in the most appropriate, least restrictive environment.
ELIGIBILITY CRITERIA:
~Must be 18 years and older.
~Individuals must have an Axis I diagnosis or dual diagnosis (Axis I and Axis II) resulting in significant functional impairments in major life activities.
Additionally, individuals must meet at least two of the following criteria:

  1. Difficult in establishing or maintaining normal interpersonal relationships to such a degree that they are at risk of hospitalization, homelessness, or isolation from social supports.
  2. Exhibits inappropriate behavior that interventions by mental health services, social services, or the judicial system.
  3. Difficulty in recognizing personal danger or recognizing significantly inappropriate social behavior.
