PROGRAM

Mental Health SkillBuilding

A goal-directed, home-based training service that is designed to assist the individual in     obtaining independence or stability within the community. The program is available to older adolescents and adults who have long term mental health needs, cognitive deficiencies or

dual diagnoses.

 

The goal of this program is to inspire positive self -change resulting in stability within the community and a healthier and improved quality of life.

 

This can be accomplished by providing individualized support to assist individuals with increasing their abilities and skills to achieve a higher level of functioning leading them to become positive contributing members of society. Services are provided in the home via goal directed training in the following areas:

PROGRAM

Addiction and Recovery Treatment Services (ASAM Level 2.1)

Substance abuse intensive outpatient programs are direct services for people with substance use disorders and/or co-occurring mental health diagnoses. The service is designed to establish psycho-social supports and facilitate relapse management and coping strategies.

 

We provide, at minimum 3 hour substance abuse groups, three times per week that encompass the process of addiction, psycho- education (anger management, conflict resolution, communication skills), and relapse prevention. The program provides between 6 to 19 hours of treatment services depending upon one's individual needs with the overarching goal for participants to maintain sobriety for lasting recovery.

PROGRAM

Partial Hospitalization (ASAM 2.5)

Partial hospitalization provides individuals (ages 18 and up) with individual and group psychotherapy, substance use disorder counseling, medication management, education groups, occupational and recreational therapy, and other therapies.

 

Program provides 5 hours per service day, with a minimum of 20 hours per week.

PROGRAM

Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is an evidence-based, intensive, team-based service for adults with severe and persistent mental illness who have significant functional impairments and a history of high service needs. ACT is delivered by a multidisciplinary team that assumes primary responsibility for assessment, treatment, rehabilitation, and support.


Services are primarily provided in the community and are available as needed, including crisis support. ACT includes psychiatric and nursing services, medication management, counseling, care coordination, and skill-building to promote stability, reduce hospitalizations, and support independent living.

Clinically Managed Low-Intensity Residential Treatment

Lighthouse offers a structured living environment with 24-hour support and integrated clinical services, tailored to individuals who need an environment anchored in stability, wellness, and recovery. The program is rooted in evidence-based practices and provides a structured framework for holistic healing and long-term recovery.
